Homelessness is a personal and societal tragedy with multiple causes. In an atmosphere that supports personal dignity and values individual strengths and self-determination, we empower homeless adults to achieve their highest potential.
Horizon House offers an array of services to address the needs of homeless adults within two PA counties: Philadelphia and Delaware. We deliver a continuum of homeless services from Outreach to Case Management and Permanent Housing Supports.
In Philadelphia, services offered include:

In Delaware County, we offer:

EMERGENCY HOUSING Services include: Eligibility: Women, 18+, with behavioral health challenges, including mental health and substance abuse. Location: Philadelphia County For more information: Referrals made through the City of Philadelphia’s Office of Supportive Housing; HOMELESS OUTREACH SERVICES Homeless Outreach teams provide engagement and resources to facilitate emergency housing, safe haven, and treatment options to homeless individuals found on the streets of Philadelphia. As part of the City’s effort to decrease the large number of homeless people in Philadelphia, Horizon House provides three Outreach Teams to travel around the streets of Philadelphia by foot and in vehicles. Teams engage the homeless in conversation day after day to develop trust, and over time, they are often able persuade individuals to accept the supports that are offered. Often the engagement process is slow because many of the individuals are dealing with issues, such as mental health and/or drug and alcohol abuse that may have contributed to their present state. During extreme weather conditions, Outreach teams engage individuals on the street with the goal of moving them to more appropriate settings, i.e., emergency housing, cafés, winter initiative beds or appropriate medical facilities, when necessary. Teams will collaborate with the police and mental health delegates to implement the Court Ordered Transportation to Shelter (COTS) and Involuntary Hospitalization (302) procedures to insure that individuals do not remain on the street during these weather conditions. Services provided: Two day-teams and one overnight team, each consisting of two individuals, assist the homeless with linkages to benefits, physical and behavioral healthcare, and alternative housing placements. Engagement, Assessment, Stabilization, and Referral (EASR)/Projects for Assistance in Transitioning from Homelessness (PATH) The goal of this program is to identify and engage shelter residents and other homeless individuals with mental health disabilities. This program will assist these individuals in accessing and managing the necessary supports for stabilization, transition to other services, and successful community living. The services provided in this program are: Eligibility: Adults, 18+ (may include related family members/children,) Location: Delaware County For more information: Contact Residential Program Director at (610)583-5830 The Navigation Center, located in West Philadelphia, provides an overnight safe haven for up to 50 homeless adults every night of the year. The City of Philadelphia funds this overnight accommodation with support services for people who are living on the streets. Supports: Eligibility: Homeless people, adults 18+ Location: PA, Philadelphia County For more information: Homeless Services, Director of Case Management – 215-386-3838, ext. 123 HOUSING Transitional Housing programs provide temporary residence, usually up to 24 months, for people experiencing homelessness. Housing is combined with case management and supportive services to assist the individual with developing stability in his/her life. The Supports to Achieve Self Sufficiency (SASS) This program is a transitional housing program located in Northeast Philadelphia. This program serves 20 homeless single men and women with serious mental illness in single and shared apartment units for up to 24 months. SASS Residential Case Managers provide case management and residential support services that include skill building in areas, such as accessing permanent housing, budgeting, shopping, cooking, apartment maintenance, and the use of public transportation. Eligibility: Homeless, single, men and women with serious mental illness who must agree to receive case management services. Individuals “graduate” from these programs to fair market housing or alternative subsidies, such as Philadelphia Housing Authority (PHA) vouchers. HOUSING FIRST TREATMENT TEAMS - HOUSING FIRST MODEL The goals of our programs are to decrease episodes of homelessness, increase quality of life, increase community integration, reduce hospitalizations and incarcerations, and assist individuals in reaching their goals. The Housing First Treatment Teams (HFTT) provide non-coercive, supportive services which are built around the “Housing First” and “Assertive Community Treatment (ACT)” models. We provide customized community services which focus on securing permanent housing quickly and then individualized services as needed. Our programs were created to serve individuals who have not been able to address their behavioral health and/or drug and alcohol issues and maintain housing through traditional treatment approaches. Our approaches are based on the Recovery Principles of an individual’s strengths, needs, and interests. What Do Housing First Treatment Teams Do? The teams provide a comprehensive array of recovery-oriented services, including: Eligibility: Location: PA, Philadelphia County & Delaware County For more information: Phila.: 215-386-3838, ext. 179 Delco: 610-583-5830 The Shelter Plus Care (SPC) program is a permanent housing program of 91 scattered site units throughout Philadelphia. This program serves homeless individuals with chronic substance abuse issues and their families with subsidized housing and case management supports. SPC allows adults in recovery from substance abuse to achieve goals toward self-sufficiency through active partnership with a program that supports their goals of sobriety, employment, and family reunification. Eligibility: Individuals should have a minimum of six months of documented sobriety and documented history of homelessness. Contact: Director, Homeless Services Case Management, 215-386-3838, ext. 123 HOMELESS SUPPORTED LIVING SERVICES The goal of this program is to provide permanent housing opportunities and flexible supports for homeless individuals and families who are diagnosed with a mental health disability. This program is designed to address the housing and mental health needs and to facilitate growth toward greater levels of independence in community living. This program will help provide opportunities for participants to connect with needed resources including psychiatric, medical, social, educational, vocational, and recreational needs. The services provided include: Eligibility: Adults residents of Delaware County that are currently homeless, persons who have a primary diagnosis of a mental health disability, and are interested and willing to participate in the program and are capable of self-preservation. All admissions require the development of a service plan that provides support for the needs of the individual. All participants must meet Delaware County Housing Authority’s Section-8 criteria and have at least one identified adult family member with a mental health diagnosis. Horizon House offers emergency housing and supports to women who are in need of these services. Randolph Court provides emergency housing for a capacity of 20 homeless women seeking a quick transition to more suitable housing. This facility, located in South Philadelphia, seeks to provide interventions that allow for stability in various life domains.
