At Horizon House we place a high value on the integration of psychiatric rehabilitation, physical health, wellness, and recovery as indispensable elements of service.
Psychiatric Rehabilitation Services
Horizon House’s psychiatric rehabilitation services promote recovery, full community integration, and improved quality of life for individuals diagnosed with any mental health condition that seriously impairs their ability to lead a meaningful life. Our services are collaborative, person-directed, and individualized. Services focus on developing a person’s skills and accessibility to resources needed for success. Our psychiatric rehabilitation services are site-based or mobile, depending on individual need.
Mobile Psychiatric Rehabilitation (MPR)
Mobile Psychiatric Rehabilitation (MPR) is a service for adults with serious mental illness. Service is provided on site, in the individual’s location of choice; their home, school, work or elsewhere in the community. Mobile psychiatry is recovery/resiliency-oriented and time limited, and is aimed at supporting individual functioning levels in four domains: living, learning, socializing and working. This service focuses on skill development and providing support for individuals to live independently in the community of their choice. Weekend hours are available as needed.

Clubhouse-Providence House
Horizon House uses the Clubhouse Model at Providence House to help adults with serious mental illness or a history of mental illness in taking an active role in their psychiatric rehabilitation. This service focuses on the personal strengths of individuals and not their illness. At Providence House, members, with staff support, make decisions based on personal preferences, abilities, and interests. Participation, while voluntary, provides members the opportunity to manage a variety of the activities needed to accomplish the goals of the clubhouse. By doing so, individuals learn cooperation, initiative, and how to develop solid interpersonal relationships.

Wellness Alliance
Community Integration and Recovery Center
The Horizon House Wellness Alliance is designed for individuals with mental illness and co-occurring drug/alcohol addiction to explore and define their own unique recovery process and develop personalized support networks for improved quality of life. Staff use psychiatric rehabilitation principles, goals, and values to assist individuals re-establish themselves in four domains: living, learning, socializing, and working. Referrals can be made via a family member, health care provider, self-referral, or through CBH (Community Behavioral Health).

Collaborative and Integrated Healthcare
Delaware Valley Community Health
In partnership with Delaware Valley Community Health and Fairmount Primary Care Health Center, the coordination of medical and behavioral health care is provided for adults with mental health needs at Horizon House, 120 South 30th Street, Philadelphia. Its focus is to support individuals in living longer, healthier, and more satisfying lives in the community. Coordination and collaboration of these services have been proven to reduce the barriers to recovery. All insurances are accepted. A sliding fee schedule is available for the underinsured and uninsured.

Health and Wellness Services
Improved health and wellness are important for enhanced quality of life. Project HEALTH was developed at Horizon House so that individuals we serve have access to primary care and to empower individuals to make healthy lifestyle choices. The focus is on preventative and primary medical care services. The Project Health team works with our Primary Care Partner, Delaware Valley Community Health’s satellite at Horizon House, to coordinate care. Project HEALTH provides individuals with tailored interventions based on their unique goals. This includes access to the wellness room, peer support, assistance navigating the health care system, and group/individuals sessions. Sessions focus on improving self-management of health conditions such as diabetes, hypertension, nutritional counseling, and tobacco recovery. Project HEALTH staff teach individuals healthy food choices, budgeting, food shopping, and meal preparation, to facilitate better health. In addition, Project HEALTH includes InSHAPE, an evidenced-based practice with a focus on physical health and nutrition. InSHAPE members have improved their cardiovascular function, have lost weight, and are spending more time in their communities.
